Athletes and parents/guardians, 

All student athletes must have both of the following on file with the Athletic Trainer before they can participate in athletic activities. The links below will take you directly to the required forms. 

1. UIL Physical Form : This form must be printed out, parents will complete the Medical History and a physician must fill out and sign the physical examination portion.

2. 2021-2022 Online Athletics Forms : Please complete the online forms and make sure to hit "Submit" on the last page. 

Physicals are good for 1 school year and will expire on the last day of school each year. If you plan to participate in the summer Speed & Strength Camp you MUST have a new physical on file for the 2021-2022 school year! 


About Me:

This is my second year at Woodridge Forest Middle School. I grew up in Stephenville, Texas where I first discovered Athletic Training when I participated as a student athletic trainer from 8th grade until I graduated. I went on to get my Bachelor's degree from Texas A&M University in Kinesiology - Motor Behavior with minors in Spanish and Psychology. After that I went on to Tarleton State University where I graduated with 2 Masters degrees, 1 in Kinesiology - Health and Wellness and 1 in Athletic Training. 

 I am excited to be a part of New Caney ISD and can't wait to see what the student athletes are able to accomplish this year!
